Massage License Requirements

Requirements for Arizona Massage License:

  • 700 transcript hours* of Massage Therapy training from an accredited massage school. You must have completed a full program.
  • National Certification from NCBTMB. (National Certification Board for Therapeutic Massage & Bodywork) Arizona USDE accredited school grads are exempt from exam (includes SWIHA)
  • $219 money order for Licensing Fee and Background Check

To become licensed in Arizona and hold out-of-state license:

  • 700 transcript hours* of Massage Therapy training from an accredited massage school. If you do not have 700 transcript hours of massage training from an accredited school, you are required to take the remaining hours of study to reach state required minimum. You must have completed a full program.
  • National Certification from NCBTMB. If you have schooled 51% or more of your 700 hours at an Arizona USDE accredited school (includes SWIHA), you are exempt from taking NCBTMB exam.
  • $219 money order for Licensing Fee and Background Check.
  • Copy of your current out-of-state license.

To become licensed for the 1st time as a graduate of SWIHA:

  • 700 transcript hours* of Massage Therapy training from an accredited massage school. You must have completed a full program.
  • You must submit to the Arizona State Board the following:
    1. Completed & notarized License Application for the Arizona Board of Massage Therapy
    2. A fingerprint card issued by the Arizona Board of Massage Therapy
    3. A money order/cashier’s check for $219 payable to AZ Board of Massage Therapy
    4. Official transcripts (sealed and stamped) showing a minimum of 700 hours
    5. Proof of U.S. Citizenship: Arizona Driver's License (issued after 1996), Birth Certificate, or U.S. Passport. Other forms of proof can be found at www.massageboard.az.gov
    6. Passport size photo taken within 60 days of applying for license

Arizona license renewal:

$95 + 25 hours of CE (continuing education) classes. These 25 hours can be anything under SWIHA’s massage programs and/or marketing classes.
